Counseling Fees

$145 / individual 60 minutes (adult and/or child/adolescent)

$175 / individual 90 minutes (adult and/or child/adolescent)

$175 / couple 60 minutes

$205 / couple 90 minutes

Payment can be made by cash, check (made out to Heart Rooted Counseling) or credit card (all accepted, including HSA).

Insurance

I am paneled with most OH Medicaid managed care plans:

  • Aetna OhioRISE Medicaid
  • Anthem BCBS Medicaid
  • Buckeye Health Medicaid
  • CareSource (Medicaid and MyCare Dual)
  • UHC Community Plan Medicaid

For all other insurances, if you wish to use Out-of-Network benefits, I can supply you with a superbill (receipt) which you submit for partial reimbursement.
